KEY RESPONSIBILITIES:

• Establish trust and rapport with program participants to understand their needs, strengths, interests, and capabilities.
• Conduct comprehensive intakes and develop service plans.
• Meet regularly with participants on your caseload, document progress towards goals, and provide ongoing support.
• Build effective partnerships with external providers.
• Maintain exceptional record-keeping and documentation of all communication and contact with participants and external partners.
• Utilize data to better plan daily and weekly tasks, contact and reschedule participants who miss appointments, and build effective communication with participants' medical and behavioral health providers.
• Represent Families Forward and Fedcap positively and professionally in the community.

MINIMUM QUALIFICATIONS FOR A CAREER SERVICES ADVISOR - EMPLOYMENT SPECIALIST:

• Bachelor's degree or higher education and/or equivalent experience
• Demonstrated track record of providing exceptional customer service
• Valid driver's license
