HOW YOU MAKE A DIFFERENCE AS AN INTENSIVE CARE COORDINATOR:

In this vital role, you provide substance abuse assessments and case management services to formerly incarcerated clients of the IMPACT program. As you welcome clients to the program, you seek to understand their unique needs and build trust through compassionate communication. With clinical judgment, you assess substance abuse barriers and develop personalized transition plans using motivational interviewing techniques. You also facilitate engaging groups and seminars on substance use topics while coordinating appointments for clients with treatment programs. WHAT WE NEED FROM YOU:

Ask yourself: Are you passionate about helping others overcome challenges? Can you effectively assess client needs and develop individualized care plans? Do you possess strong communication and interpersonal skills? If you answered "yes" to these questions and meet the following requirements, we need you as our Intensive Care Coordinator!

• Bachelor's or master's degree in social work or a relevant field, such as psychology, vocational rehabilitation, or occupational rehabilitation
• Credentialed Alcoholism and Substance Abuse Counselor (CASAC) certification
• Strong writing skills

Being bilingual in English and Spanish is a plus!
