As a Case Manager II - Human Services Case Management Specialist, your role revolves around dedicated advocacy and comprehensive support for individuals enrolled in the CLASS Medicaid Waiver program. Your responsibilities encompass conducting thorough assessments, crafting individualized service plans, continuous progress monitoring, and facilitating informed decision-making regarding housing, employment, and personal aspirations for your clients. Your dedication fosters positive transformations in the lives of those you serve. Furthermore, you will maintain collaborative relationships with Direct Service Agencies, enhancing service delivery. Embark on this fulfilling journey and be the change you aspire to see! All that you need is the following:

  • Master Social Worker license in the state of Texas, OR a master's degree in human services, drug and alcohol education, counseling, psychology, or criminal justice, OR a bachelor's degree in human behavioral science (30 semester or 45 quarter hours either in the development of human behavior, child development, family intervention techniques, diagnostic measures, or therapeutic techniques
  • 2+ years of experience as a CLASS Case Manager
  • 2+ years of experience providing services to children or adults with disabilities
  • Knowledge of Microsoft Office products or the ability to learn them
  • Strong communication skills
  • Ability to work independently and with minimal supervision
  • Highly analytical with extensive critical thinking skills for creating client plans
  • Basic mathematical abilities
  • Ability to travel throughout the North Texas area
  • Valid driver's license and proof of insurance

Spanish/English proficiency is preferred but not required.
