YOUR MISSION
As a Mental Health Specialist - Case Manager, you play a crucial role in supporting the clients in our IMPACT program. Your day-to-day responsibilities include conducting screenings and intakes, informing clients about program goals and regulations, and collaborating with a compassionate interdisciplinary team. You develop individualized service plans using a person-centered approach and utilize motivational interviewing techniques to empower clients.
You determine the appropriateness of the care being provided, review treatment history, and facilitate access to suitable treatment programs. Additionally, you are a source of support for clients' families and liaise with community-based agencies. Your compassionate and caring approach makes a lasting impact on the lives of those you serve.
WHAT YOU NEED TO GET STARTED
Bilingual fluency in English and Spanish would be preferred.
