A DAY IN THE LIFE OF A CASE MANAGER
As a Case Manager with our nonprofit, you're responsible for conducting all required case management services for our clients within an established timeframe. You maintain current and accurate documentation of evaluations, assessments, needs, progress, services, financial data, and other relevant categories in each individual's records. Subscribing to our philosophy of independent living, empowerment, and community integration, you ensure clients are able to make informed decisions regarding important life aspects, including housing, employment, relationships, education, and other personal goals.
In addition, you regularly travel to meet with clients to discuss their progress, concerns, and plans to ensure their actions align with their individual service plans (ISPs). You also maintain a cooperative working relationship with direct service agencies (DSA) to enhance our service delivery and assist participants in achieving their personal goals. As needed, you quickly respond to phone calls and make sure clients have access to case management services in emergency situations. You complete monthly program reports and billing forms as well as maintain updated knowledge of community resources and state or federal programs as they relate to client needs. You enjoy having a job that uses your psychology and counseling skills to do important work, which is why you thrive in this social work position!
QUALIFICATIONS FOR A CASE MANAGER
Bilingual in English and Spanish is preferred, but multiple factors will be taken into consideration. Do you have excellent verbal and written communication skills? Can you use analytical and critical thinking skills to create client plans? Are you able to work independently with minimal supervision? Can you handle confidential information with discretion?
