YOUR LIFE AS A SOCIAL WORK CASE MANAGER

This full-time case management social worker position in human services works during the day.

As a Social Work Case Manager with our nonprofit, you're responsible for conducting all required case management services for our clients within an established timeframe. You maintain current and accurate documentation of evaluations, assessments, needs, progress, services, financial data, and other relevant categories in each individual's records. Subscribing to our philosophy of independent living, empowerment, and community integration, you ensure clients are able to make informed decisions regarding important life aspects, including housing, employment, relationships, education, and other personal goals.

In addition, you regularly travel to meet with clients to discuss their progress, concerns, and plans to ensure their actions align with their individual service plans (ISPs). You also maintain a cooperative working relationship with direct service agencies (DSA) to enhance our service delivery and assist participants in achieving their personal goals. As needed, you quickly respond to phone calls and make sure clients have access to case management services in emergency situations. You complete monthly program reports and billing forms as well as maintain updated knowledge of community resources and state or federal programs as they relate to client needs. You enjoy having a job that uses your psychology and counseling skills to do important work, which is why you thrive in this social worker case management position!


WHAT WE NEED FROM YOU AS A SOCIAL WORK CASE MANAGER

  • Licensed Social Worker in the state of Texas OR master's degree in human services, drug and alcohol, education, counseling, psychology, or criminal justice
  • 2+ years of documentable experience providing services to children or adults with disabilities
  • Working knowledge of Microsoft Office products, including Word, Excel, and Outlook OR the ability to learn independently
  • Ability to perform basic math computations at a 6th-grade level minimum
  • Ability to travel through north Texas to visit clients
  • Valid driver's license and proof of insurance
  • Covid-19 vaccination
